Tri-Care is insurance for many Federal and Military retirees. Due to the lack of health care facilities for veterans, Tri-Care members are afforded access to home care services within the Commonwealth. With the nearest Veteran’s or Military Hospital being on Guam, Tri-Care members do not need a pre-authorization to initiate home care services.
To have home care services you need to do the following:
1. Request from your physician at the hospital or clinic to refer you for home care services, and to MHS Home Care.
2. Either you or your physician should fax the referral to MHS Home Care’s office or you can deliver during regular business hours.
3. Contact MHS Home Care at 233-4646 so our staff can begin coordinating with your physician for your discharge from the hospital or from the clinic.
4. Prepare all your insurance information.
