Currently MHS Home Care accepts and coordinates services from StayWell, Moylan’s NetCare, Calvo’s Select Care, AON Insurance, Worker’s Comp Insurance, and many others. To see if you’re insurance covers home care services, and if MHS Home Care accepts your insurance please call your health insurance representative.
For the mentioned consumer health insurance a “pre-authorization” is required before home health care services can commence. Each health insurance provider also has differing coverage for home care, please check your benefit listing or call your health insurance representative for more information.
To have home care services you need to do the following:
1. Request from your physician at the hospital or clinic to refer you for home care services, and to MHS Home Care.
2. Either you or your physician should fax the referral to MHS Home Care’s office or you can deliver during regular business hours.
3. Upon receipt MHS Home Care will immediately fax a “Pre-Authorization” request along with the physician signed referral form to your health insurance provider
4. Contact MHS Home Care at 233-4646 so upon approval from your health insurance provider our staff can begin coordinating with your physician for your discharge from the hospital or from the clinic. 5. Prepare all your insurance information.
