Medicare recipients qualify for home care services if they meet one of the following:
1. Are 65 years or older; OR 2. Are disabled for at least 2 years and are under 65 years (based on number of quarters contributed to Social Security…need to check with Social Security Office); OR 3. Have End Stage Renal Disorder (ESRD) / Kidney Failure / On Dialysis
Under the Medicare Program home care is covered for those recipients who have Medicare Part A, Part B or both.
Upon a doctor’s referral, MHS Home Care’s professional staff will work with you, the patient, your physician and your family to ensure that the best quality care is provided to get you back to your normal activities.
To have home care services you need to do the following:
1. Request from your physician at the hospital or clinic to refer you for home care services, and to MHS Home Care.
2. Either you or your physician should fax the referral to MHS Home Care’s office or you can deliver during regular business hours.
3. Contact MHS Home Care at 233-4646 so our staff can begin coordinating with your physician for your discharge from the hospital or from the clinic.
4. Prepare all your insurance information.
