Medicaid recipients now have the opportunity and access to continuous quality health care upon discharge from the hospital or visit to the hospitals’ outpatient clinic. Home care services are unlimited for Medicaid recipients and are based upon the allowable cap of the Medicaid Program. This allows our Medicaid clients to receive the best care to assist in their recovery and rehabilitation to their previous normal state.
Medicaid recipients qualify for home care services if they meet one of the following:
1. Is a current recipient of the Medicaid Program
2. Possess a valid Medicaid Card
3. A physician determines a medical necessity for continuous care at home
4. A physician signed referral form is received by MHS Home Care
Medicaid recipients do not need pre-authorization to have services rendered to them by MHS Home Care. Therefore, the continuous care provided to Medicaid recipients may commence immediately within 48 hours after MHS Home Care receives a physician signed referral form.
To have home care services you need to do the following:
1. Request from your physician at the hospital or clinic to refer you for home care services, and to MHS Home Care.
2. Either you or your physician should fax the referral to MHS Home Care’s office or you can deliver during regular business hours.
3. Contact MHS Home Care at 233-4646 so our staff can begin coordinating with your physician for your discharge from the hospital or from the clinic.
4. Prepare all your insurance information.
