Do I Qualify?

At MHS Home Care, we understand that dealing with the illness and recovery of a love one can take an emotional toll on the entire family. That is why our professional staff are there to assist in making the recovery process easier to deal with and prepare for.

With MHS Home Care’s personalized and individual care, we work directly with you and your family to find out and explain what insurance is available to you, what is covered, what is your co-payment, and explain and prepare with you the discharge and transition into your home.

Those who qualify are referred by a physician in a clinic or at the hospital and who is either:

  • Sick or Disabled due to an illness
  • Recovering and rehabilitating from surgery
  • Has a Chronic and/or Terminal Illness
  • In need of additional Skilled Nursing Services
  • In need of outpatient Physical Therapy Services
  • Homebound/Bedridden and cannot receive outpatient services because of transportation and/or other factors

As Medicare Certified Home Health Care Agency, MHS Home Care can also provide services to those who are:

  • Turning 65 years or older or
  • Disabled for 2 years and Under 65 year or
  • Have End-Stage Renal Disease (Premature Kidney Failure)

When you qualify for service MHS Home Care will work consistently with your referring physician to ensure that continuous and proper care is provided to you. What you have to do to get professional services in your home are:

  • Request that your physician at the Clinic or the Hospital refers you.
  • Contact MHS Home Care so we can begin working with you and your family to explain and prepare for the discharge process.
  • Find out what insurance you have and what it covers.
